In this position, you will assist patients with their daily living needs, provide compassionate care, and generally increase their quality of life. Responsibilities may include monitoring vital signs, assisting with hygiene needs, and communicating with nurses and doctors.

Continuously make resident care rounds, placing side rails, reposition residents, encouraging and alleviating distress using proper methodology. Assist with toileting via bedpan, urinal, bedside commode, or to the bathroom, following toileting schedules.

p>What You''ll Be Doing:

p>Responsibilities: • Respond promptly to resident call lights and assess needs • Assist residents with personal hygiene and daily activities including bathing, dressing, toileting, and mobility • Reposition and turn bedridden residents to prevent bedsores • Monitor and support residents' dietary needs, including restrictions and preferences • Provide emotional support and companionship to residents and families • Maintain a clean, safe, and supportive environment • Perform additional duties as required or directed. In this vital support role, you will assist residents with their daily care needs under the supervision of a nurse, helping to ensure their comfort, dignity, and well-being.

Under the direction of the Center Manager and/or Team Lead/Home Care Coordinator the Participant Care Associate (PCA), provides supportive services to the participant; including but not limited to in the home or community setting, during transport and on appointments, or and in the day health center, under the supervision of the registered nurse.

The PCA is responsible for accurate timely documentation as required by the service area they are supporting; including but not limited to the care and/or services provided on every visit and or encounter, transport and appointment, on the PCA/CNA progress note; the PCA keeps the RNCM and or clinic nurse informed of the participants progress.

p>Position Purpose:

Our HHA/CNA provides paraprofessional services to the organization's home care clients as developed by the primary nurse and as prescribed by their physician and in compliance with the state's Nurse Practice Act, and any other regulations defining home health aide / hospice aide services.
