Working within a multidisciplinary care team, the Nurse Practitioner will provide high-quality, patient-centered care with a particular focus on improving access and health outcomes for underserved communities on Chicago’s South Side. This individual will collaborate closely with physicians, advanced practice providers, behavioral health professionals, and other members of the care team to support the organization’s whole-person approach to care.
