Services: Deliver advanced wound care services, including assessment, debridement, and development of individualized treatment plans Provide mobile care in patient homes, assisted living facilities, and partner locations Diagnose and manage chronic and acute conditions using a comprehensive, whole-patient approach Document all patient encounters, treatments, and outcomes accurately within the EMR system Collaborate with care teams, facility staff, and referral partners to ensure continuity of care Maintain compliance with Medicare guidelines, state regulations, and clinical protocols Perform additional services within scope of licensure as agreed upon in contractor agreement .
