Facilitate and coordinate care with other providers, including home health, primary care, specialists, skilled nursing facilities, assisted living facilities, caregivers, and other members of the patient's healthcare team, through the EMR and approved digital communication modalities to enhance care and prevent and manage complications. Roles and Responsibilities: Provide in-person evaluation and management visits for patients with acute and chronic wounds and related skin disorders within a defined geographic area traveling to different clinic locations throughout the week.
