Point Of Care visits: wound care, wound vac, IV therapy (antibiotics, TPN), post ortho/joint, incision assessment follow up, post surgical assessment/complications, post stroke, pneumonia; patient education meds, wound care; trach care, teaching; GTubes feedings, teaching; ADLs assessment, OASIS Doc Exp. Yes, clinicians are assigned to a POD which consists of a group of zip codes and a multidisciplinary Team to manage the patients, however if needed they will assist in other PODs to provide patient care.
