Roles and Responsibilities Include: ASSESS, MONITOR, AND ASSIST WITH INDIVIDUAL'S HEALTH & MEDICAL NEEDSComplete initial nursing assessments and Discharge summaries for DD Waiver and ICF-IID Completion of Monthly / Quarterly / Semi-Annual nursing reports to HCC and IDT, distribution via scomm to GL Admin and IDT members, and attached to appointment in Therap. Review in Therap and communicate to HCC ALL: Medication Changes, Appointments Needs, Health Tracking and Vitals NeededMedical concerns documented in T-logs or ISP DataProvide follow-up regarding GERs that involve Individual's health and medical issues, including the use of ER, urgent care, and/or EMT services; injuries; medication errors.
