Deliver and document nursing interventions as agreed upon in the patients' care plans, including maintaining a healthy and safe environment, responding to physician orders, administering medications, and performing tests and treatments. They will work in close coordination with the Physician, RCFE facilities, clinic staff, and other interdisciplinary team (IDT) members to manage ongoing care, complex clinical situations, and support the operation of the primary care clinic.
