POSITION HIGHLIGHTS • Be the primary point of contact for Inpatient Care Management • Perform care coordination tasks required to successfully run different programs in care management that will make it easier to move patients to the appropriate level of care • Serve as the keystone for care coordination within the department, the health care team including: physicians, advanced practice providers, RNs, rehabilitation specialists, social services and external agencies • Identify the clinical targets and milestones for discharge and lead efforts to eliminate barriers, as needed • Ensure effective communication of the transition plan to all care team members.
