The Physical Therapist will also assess in conjunction with Occupational Therapists seating systems, therapeutic equipment and supports to promote mobility, optimal functioning, and safety and falls prevention, and complete comprehensive evaluations/assessments based on person center planning, initial Physical Therapy plans (Objective Plans, Baselines, and Data Sheets), progress notes and discharge notes as specified by departmental/center policies and procedures and current licensure requirements. The Physical Therapist will provide assessments and treatments per departmental and center policies/procedures for assigned caseload to include screenings, evaluations, consultations, planning and implementing Physical Therapy treatments and programs utilizing an interdisciplinary team process, or when a medical event has occurred requiring acute skills Physical Therapy services.
