• Minimum commitment of 3 weekdays per week.
$200,000 - $350,000 a year
In this role, you will provide advanced palliative care to patients in their homes, helping them achieve the highest quality of life possible while managing complex medical needs. You will work autonomously in collaboration with the Palliative Medical Director, delivering exceptional care and guiding patients and families through every stage of treatment.

p>As a Nurse Practitioner, you initiate, develop and implement nursing care plans in accordance with state laws and concurrent with Concentra medical protocols in accordance with Concentra policies, practices and procedures and applicable. Performs ancillary testing and tasks as ordered by the Center Medical Director (PFT, EKG, injections, audiograms, blood draws) in accordance with established policies and procedures.
Evaluate patients for the presence or absence, severity and location of peripheral vascular disease by selecting the most effective procedure(s) from an array of anatomic, hemodynamic and physiologic procedures including duplex scanning, plethysmography, doppler ultrasound, pressure measurements and active or passive hyperemic challenge. As our Sonographer, you will utilize advanced sonographic equipment to record detailed images of blood, organs, tissues, and bodily masses, including unborn fetuses, providing critical visual data for diagnosis and prenatal care.
