Tri-Area Community Health (https://triareahealth.org/) is a Federally Qualified Health Center headquartered in Laurel Fork, Virginia, with community clinics in Cana, Ferrum, Floyd, Fries, Grayson Highlands (Troutdale), and Laurel Fork, Virginia along with current school-based health centers in collaboration with Carroll County Public Schools, Galax City Public Schools, Grayson County Public Schools and planned collaborations with Floyd County Schools and Patrick County Public Schools. Our three large sites are National Committee for Quality Assurance-certified Patient-Centered Medical Homes and medical professionals at all sites are eligible for National Health Service Corps Loan Repayment (HPSA score of 19).We are excited to be opening a new school-based clinic in collaboration with Patrick County Public Schools.
