To be successful in this role, you will review and verify medication orders for appropriateness, safety, and efficacy, considering patient-specific factors such as age, weight, renal function, and allergies; monitor serum drug concentrations and adjust dosages as needed to achieve therapeutic goals and minimize toxicity. As our Clinical Pharmacist, you will provide comprehensive pharmaceutical care services to patients, working collaboratively with physicians, nurses, and other healthcare professionals to optimize medication therapy and improve patient outcomes.
