Demonstrate knowledge and skills necessary to provide care based on physical, psychosocial, education, safety and age related criteria pertinent to the patients served in his/her assigned area that may include, but not limited to, pharmacokinetic evaluation and monitoring, patient's chart review, antibiotic monitoring, automatic renal dose adjustments, IV-PO switches, warfarin consultations, and therapeutic interchange. In this role, the successful candidate Responsible for safely preparing, compounding, and dispensing medications to patients in accordance with the laws and regulations governing pharmacists and the practice of pharmacy by the NYS Board of Pharmacy and NYU Langone Health- Brooklyn's pharmacys policy and procedure.
