With 12 medical and dental locations across Lucas County, we offer comprehensive services - including pediatrics, adult medicine, dental care, women's health, healthcare for the homeless, senior care, behavioral health, pharmacy, and lab services. All members of the PCMH Care Team will work cooperatively with and interact with the patients and other team members to coordinate patient care and achieve the stated patient and PCMH Team goals and objectives.
