Perform in-home (and occasional virtual) health assessments, including medical history reviews, physical exams, screenings, and medication reviews • Conduct thorough evaluations of chronic and acute conditions • Utilize pre-populated clinical templates to support efficient, accurate documentation • Submit chart notes within required timeframes and address queries promptly • Maintain clear communication with clinical leadership and operations teams • Provide a self-managed schedule with a minimum monthly availability expectation (varies by market) • Offer appointment availability across days, evenings, and weekends • Deliver empathetic, patient-centered care in a respectful and professional manner • Educate members on whole-person wellness and connect them to appropriate resources. • Active, unrestricted NP or PA license in the state where assessments will be performed • Board certification (ANCC, AANP, or NCCPA) • New York candidates: Also accepting board-certified or board-eligible MDs • Current BLS certification • Valid driver's license, reliable transportation, and auto insurance • Ability to pass all background checks-including criminal, SSN validation, education, employment verification, healthcare sanctions, sex offender registry, and drug screen • Strong clinical assessment and diagnostic skills • Proficiency with EMR systems and comfort using digital tools • Ability to lift and carry up to 20 lb • Mental and physical stamina to perform assessments and documentation.
