Develop and maintain strategic referral relationships with community organizations/providers, service coordination entities, MCO’s, Veterans groups, Assisted Living facilities, physicians and other healthcare professionals and the general public who refer clients and their families to in-home care services to build JCAH network and increase JCAH’s presence in the community. The Care Manager, Recruitment will identify and work with community providers, healthcare organizations, service coordination entities, Veterans assistance groups and the general public to grow and maintain the Managed Care Organization, Private Pay, VA Business lines of business.
