Job Description: GENERAL OVERVIEW: As a member of the health care team, assists with population health management through patient outreach and coordination of care, participates in the clinical visit to assist the provider, and provides self-management support to patients. Accurately performs clinical transcription during the patient encounter by documenting elements of the clinical visit under direct supervision of the provider and assisting with computerized provider order entry under direct supervision of the provider.
