This role supports the Behavioral Health outpatient services program through the participation on an inter-disciplinary team including prescribers, clinical supervisors, nurses, community-based specialists, pharmacy staff, and administrative staff to provide services, collaborate with external providers, and incorporate health and wellness needs into the plan of care of clients served. Models and reinforces Community Connections mission to provide behavioral health, residential services, and primary health care coordination for marginalized and disenfranchised women, men, youth, and children living in the District of Columbia, many of whom are coping with challenges including mental illness, addiction, and the aftermath of trauma and abuse.
