Works collaboratively with leaders, physicians, referral sources, and caregivers across Hospice, Home Health, and the broader health system to improve communication, streamline workflows, and support continued growth and excellence in post-acute care services. This role will initially oversee Hospice Intake for the East region and serve as a key leader in the strategic alignment and integration of Intake services across additional Hospice communities, with future expansion to include Home Health Intake.
