Hospice Triage RN provides patient physical and psychosocial health systems assessments telephonically, supporting the Plan of Care, “Goals of Care” based on the patient’s and caretaker’s needs/wishes, and in coordination with plan developed/placed by the primary RN Case Manager for the patient. The ONSITE Call Center Triage RN is responsible for the emergent triaging of calls received from patients and/or caretakers to for the purpose of telephonically assessing their needs in order to treat emergent situations that have developed during the Call Center hours .
