Under the direction of a Registered Nurse (RN), the Licensed Practical Nurse delivers efficient and effective nursing care while achieving positive clinical outcomes and patient/family satisfaction.
*Evaluate patients/residents to collect data, observe condition and report changes in condition.
Social workers provide care in both the inpatient and ambulatory settings, including the Emergency Department, with a focus on completing psychosocial assessments, providing resource referrals, and participating in care coordination and discharge planning for patients across the continuum of care.
