Provide services within the scope of practice as defined by the state laws governing the practice of nursing, initiates and coordinates the plan of care, collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily. Complete supporting documentation accurately and timely, including visit report, OASIS assessment, plan of care and revisions, medication profile, progress notes, recertification, discharge/transfer summary, etc.
