RN Field Case Manager: The Field Registered Nurse (RN) Case Manager is responsible, as a member of the interdisciplinary team, for the provision of skilled nursing assessment and hospice care to maximize the comfort and health of patients and families/caregivers consistent with Lightways Hospice and Serious Illness Care policies and procedures.

Acts as Liaison for patients, families, and their multidisciplinary clinical team to address the population health needs of our patients by assessing for clinical, emotional, spiritual, psychosocial, and financial barriers to care; Once identified, works with care team to find a plan to overcome barriers for successful health outcomes.
