Work consists of case management, starts of care (SOC), recertifications, resumptions of care (ROC), and other assessments and services, as well as occasional new hire orientations, in-service and supervisory reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall. To support our growth and the increasing volume of client care throughout Ohio, we are seeking a highly skilled, experienced, accountable, and self-motivated home health RN Case Manager (RNCM) to join our agency and serve existing and returning clients throughout Stark County and nearby counties across Northeast Ohio in a part-time or full-time capacity.
