The navigator collaborates closely with the HF Program Coordinator, Critical Care Unit Coordinator, physicians and advanced practice providers, care coordinators, social workers, clinic staff, Community Paramedicine teams, and Clinical RX-2-Go Pharmacists to ensure seamless care delivery across inpatient, outpatient, and community settings. Under the guidance of the East Region Performance Improvement Manager, the HF Nurse Navigator supports the Heart Failure Program through proactive patient identification, care coordination, education, and performance improvement initiatives aimed at improving clinical outcomes and reducing preventable utilization.
