We are seeking a Clinical Scheduler to fill our Care Manager role assist with all clinical scheduling needs for our Myrtle Beach patients. A successful Care Manager is responsible for coordinating all in-person facilitator visits and in-person provider encounters for patients assigned to their care team. Through effective scheduling and coordination, the Care Manager ensures that patients receive consistent monitoring, engagement, and care team oversight. Your Health’s model of care is based on frequent patient engagement, coordinated interdisciplinary services, and proactive monitoring, which has consistently been shown to reduce hospitalizations and improve patient outcomes.The Care Manager works closely with Directors of Population Health (DPH), providers, facilitators, and specialty population health divisions to ensure that clinical recommendations are translated into a clear and organized patient visit schedule.The following is a list of essential functions, which may be subject to change at any time and without notice. Management may assign new duties, reassign existing duties, and/or eliminate function(s)Areas of Responsibility:<ul><li>Appropriately register, verify and timely schedule patients according to the Company’s care model.</li><li>Ensure new patients are being tracked and followed until scheduled.</li><li>Schedule appointments for patients according to care model including follow-ups with healthcare providers based on availability, medical urgency, and patient preferences.</li><li>Ensure all disciplines with schedules are fully optimized.</li><li>Ensure patients are being followed and scheduled appropriately when discharged from an in-patient facility.</li><li>Ensure TCM appointments are scheduled promptly and within the required window.</li><li>Ensure strong documentation is implemented.</li><li>A multitude of software systems (including tracking systems) will be utilized to ensure patient follow-up and better patient outcomes.</li><li>Interact with patients via email, patient portal, telephone, AthenaText, in-person, etc. to gather 
	necessary information, schedule and confirm appointments, and provide pre-appointment 
	instructions.</li></ul>Qualifications <ul><li>High school diploma or equivalent required, while an Associate's or Bachelor's Degree in Healthcare is preferred.</li><li>Preferred CMA, LPN or RN</li><li>Previous experience in a medical or healthcare setting, preferably a scheduling or administrative role.</li><li>Familiar with medical terminology, procedures, and insurance verification purposes.</li><li>Strong organizational and time management skills, with the ability to multitask and prioritize 
	responsibilities effectively.</li><li>Excellent communication and interpersonal skills with the ability to build strong relationships at all levels of the organization.</li><li>Strong problem-solving abilities</li><li>Ability to work effectively under pressure.</li></ul>

As an HVAC Dispatcher, you will coordinate daily scheduling and dispatch operations for residential HVAC service technicians.Responsibilities include:<ul><li>Dispatch HVAC service technicians efficiently to meet customer appointment times</li><li>Manage incoming service requests and schedule heating and air conditioning service calls</li><li>Coordinate technician routing and job assignments to maximize efficiency</li><li>Monitor call flow through the HVAC service platform and dispatch system</li><li>Communicate with technicians and customers to provide updates on HVAC service appointments</li><li>Support field teams throughout the day by adjusting schedules and resolving routing issues</li><li>Maintain accurate service scheduling and ensure technicians remain productive throughout the day</li></ul>This is a fast-paced, in-office dispatcher position supporting residential HVAC service operations.

Qualifications:
