Initial contact with families whose children have been referred to the Infant Toddler Program Conduct intake meeting with families-completing required paperwork and initial assessment Request records as needed for service provision Work with interdisciplinary team to insure referred children are evaluated for eligibility Write Individual Family Service Plans (IFSPs) for eligible children Conduct reviews of IFSPs at least every 6 months Connect families to community resources and supports Support families with transition out of the Infant Toddler Program, including to local school district programs Complete data entry as required Participate in required program trainings Collaborate with team members on shared cases.
