In addition, responsibilities include interacting with physicians, members/families, complex case managers, home care reviewers, inpatient care coordinators, medical office staff, and other Kaiser Permanente staff, member/family and others, and promotion of a high level of member satisfaction. Identifies, integrates, and coordinates Kaiser Permanente and community health care resources and services for members and works in conjunction with utilizing Thrive Local (Community Support Hub).
