Care Manager Responsibilities:• Provide case/care management to Medicaid Members seeking assistance from Salvation Army Centers, including screening, assessment, social care plan, and direct service delivery related to specific health-related social needs as identified by Social Care Network.• Manage incoming referrals for enhanced HRSN services, within required timeframes, to ensure successful and timely connections are made for community members.• Conduct eligibility assessments and refer Members to eligible programs and services, including enhanced HRSN services (Level 2) and/or existing federal, state, and local resources (Level 1).• Develop social care plans that include a summary of Member needs, eligibility, and services to which they are referred.
