The JSC provides ongoing case management to juveniles committed to IDJC and placed in either state or private facilities, collaborates with respective facility case managers as well as other members of the treatment team, including family and guardians, the juvenile probation officer and other treatment providers critical to the juvenile's successful return to the community. Juvenile Services Coordinators may conduct home visits prior to a juvenile's release from Department custody in an effort to work with families, county probation, and community resources to assess current needs, and collaboratively develop plans to support the juvenile's successful community re-entry.
p>Interventions may include but are not limited to brief counseling, supportive therapy, crisis intervention, grief counseling, psychiatric placement/referral, alcohol/drug assessment and referral, and referral to alternative community resources. Participates in care coordination activities for those patients identified as needing social work and/or complex case management interventions on a well-developed, close-knit, 10-person team.
