Provide comprehensive outpatient primary care services for patients of all ages (FM) or adult population (IM)• Diagnose and manage acute and chronic conditions including diabetes, hypertension, asthma, COPD, and heart disease• Perform preventive care, annual wellness visits, and health screenings• Coordinate care with specialists and ancillary services• Utilize EPIC EMR for documentation and patient management• Participate in quality improvement and value-based care initiatives• Collaborate with Advanced Practice Providers and multidisciplinary care teams.
