This position supports the coordination of care for youth involved with the Child and Family & Community Mental Health Enhancement (CME) program through hospital partnership development, care coordination, service connection, and admission/discharge collaboration to help ensure timely access to services and continuity of care. Services ranging from counseling, pharmacological management, primary care, psychological testing, case management, substance use treatment, residential services, vocational program, and more!
