Duties and responsibilities may include: Assist with case management and rehabilitation services using a strengths-based, welcoming, recovery-oriented, client-driven approach for an assigned group of clients, Observe the development of strengths-based comprehensive intake screening, assessment; treatment plans; modification of treatment plans. JOB SUMMARY: Under close supervision, responsible for assisting with Individual and/or group services, which include assessment, coordinating, planning, advocacy, linkage, referral, education, crisis intervention, resource acquisition, and monitoring in a culturally sensitive way.
