The Resident Care Manager I shall coordinate the participant’s care in collaboration with the participant, and all members of the participant’s care team (e.g., primary care physician, mental health service provider, substance use disorder service provider, ECM providers, housing navigators, street outreach case managers, family members, etc.) and support the day-to-day operations of the Community by performing the following duties: Works with Community Property Management to provide service linkages and support systems to residents to assure individual needs are met. Ability to perform under circumstances of possible emotional stress and conflicts dealing with difficult, uncooperative, and potentially aggressive clients as VOALA serves the neediest of the needy in our programs which may include clients with homelessness, mental-illness, substance abuse, recently released from incarceration and other challenges.
