The Director of Transition to Community Living and Care Management (TCM-CM) is responsible for the leadership, operational oversight, implementation, and performance of care management services supporting members with complex behavioral health, physical health, intellectual and developmental disabilities, traumatic brain injury, serious mental illness, severe substance use disorder, and housing-related needs. The Director is responsible for translating strategic priorities into operational execution, developing leaders, monitoring program performance, supporting budget stewardship, and advancing whole-person care models that improve member outcomes, promote community integration, and support housing stability.
