This position supports client engagement throughout the continuum of care by coordinating appointments, referrals, transportation, discharge planning, communication with referral sources and outside providers, and collaboration with the interdisciplinary treatment team. The Case Coordinator assists clients in accessing health, behavioral health, social, and community resources while promoting continuity of care, treatment engagement, and successful recovery outcomes.
