li>Work with the treatment team on assessing treatment needs, support development and monitoring, provide consultation to mental health aides, and transition planning -including completion of referrals, attending screenings, advocacy, maintaining housing, obtaining resources, benefits and consultation regarding client needs.
Environmental: Work is performed in one or more of the following: clinical office, acute care settings and community setting including client residences, correctional facilities, shelters, hospitals, public places and other social service sites.
