Principle Responsibilities Care Coordination, Clinical Management, and Transitions Across the Continuum Coordinates care for adult transplant and cellular therapy patients across the full cancer care continuum, including pre-treatment, treatment, post-treatment, survivorship, and long-term monitoring. Serves as the primary clinical and communication resource for Transplant and Cellular Therapy patients, facilitating progression through evaluation, workup, treatment planning, and therapy while ensuring timely information exchange among patients, providers, and multidisciplinary team members.
