Key Responsibilities: Manage daily operations and staff in Care Management Department, including care coordination, case management, discharge planning, utilization management, social work services, and pre-admission case management. Act as liaison between patients, families, department staff, physicians, healthcare team, community agencies, managed care representatives, and skilled nursing facilities.
