Qualifications:

• BA/BS Degree preferred • A minimum of 2 years of experience working with the I/DD population or other experience with criminal justice systems • Experience in Service Coordination/Case management or Intake/Admissions preferred • Must be able to communicate clearly and effectively, verbally and in writing, in order to best represent YAI. • Must be able to work autonomously and work well in groups. • Must be proficient in basic computer skills including Microsoft Office 365 and the applications; word processing and data entry; • Able to learn and adopt to many database systems • Highly organized, good time management skills and ability to prioritize multiple tasks.

POSITION DESCRIPTION

• Work with CVRC, PDC and other RCs across CA to request required documents for assessment and enrollment; • Collaborate with program management throughout screening process and ensure adherence to screening process timeframes; • Maintain data and documents in shared drives and appropriate database systems; • Verify each applicant's eligibility and ensure necessary enrollment codes are present upon admission; • Maintain linkage database with community partners (PDC, DDS, RCs, community providers and County Behavioral Health.) • Participate in outreach and community-based meetings to network, complete and disseminate minutes • Manage and maintain client files in compliance with DDS, Regional Center and YAI guidelines • Complete other tasks based on departmental and regional needs and as assigned by supervisor.

Hourly rate: $24.04 - $24.04
