Experience in case management, care coordination, social services, behavioral health, recovery support, reentry services, or human services (Two or more years of relevant case management, care coordination, reentry, recovery, or behavioral health experience preferred). Working closely with members and multidisciplinary treatment teams, this position helps identify needs, develop individualized plans, coordinate services, and reduce barriers related to housing, healthcare, benefits, transportation, employment, education, and financial stability.
