Develop and maintain relationships with courts, probation, community supervision agencies, behavioral health providers, substance use treatment providers, recovery organizations, housing providers, workforce organizations, social service agencies, faith-based organizations, and other community partners to improve communication, coordination, continuity of care, and successful transition from incarceration to the community. Coordinate services and operational activities for the jails Medication-Assisted Treatment (MAT) program in collaboration with the jail physician, nursing staff, behavioral health clinicians, community treatment providers, recovery organizations, and other partners.
