With a patient-centered approach the Medical Director will see a small panel of their own patients, provide clinical oversight and hands-on coaching to Upperline Plus' nurse practitioners, identify market opportunities, improve clinical processes, and lead VBC and population health initiatives - all with a focus on optimizing the care model to improve patients' outcomes and quality of life. Our providers coordinate patients' care and collaborate across a multidisciplinary team - including physicians, advanced practice providers, care navigators, pharmacists, dieticians, and social workers - to address both immediate and long-term patient needs.
