Support services include case management and behavioral health counseling, referral to and coordination with primary medical care, substance abuse and psychiatric treatment referrals, benefits counseling and client advocacy, meal programs, health education, community building, tenant organizing, and all other case management functions. Under supervision of the Program Director, the case manager participates in a multidisciplinary team, including external and internal agency team members consisting of mental health clinicians, primary care providers, nurses, peer specialists, interns, and psychiatric medication providers.
