Under the direction of the CSD, AVP of Care Delivery, RED, or based on the needs of any site, the Regional Rehab Manager will visit community resources (SNF, MD offices, Out-PT Rehab clinic) to assess PACE members, Discuss POC's, develop business partnerships or to promote PACE services. The Regional Rehab Manager is responsible to ensure a high level of therapy is provided to all PACE members in order to have positive outcomes including low hospitalization rates, low fall rates, and that all members function at their highest level of ADL independence.
