We are looking for a proactive employee to use their own experience and judgment to carry out the mission of DD/ABI Community Services, including: Community Outreach (including Comm Hab, Outreach, and TRAID), Day Habilitation, and Self-Direction. o Establish and maintain a rapport with OPWDD, CNY Developmental Disabilities Regional Office (DDRO), MCOs, Children’s Health Homes, and other service providers; participate in external audits (OPWDD) and exit conferences.
