Under general supervision, to perform specific program care coordination, group facilitation, and a variety of community and health education activities in the Health Care Agency (HCSA) case management, care coordination, home visiting and/or family support programs and work collaboratively to provide a seamless system of services and support for clients in Alameda County; acts as a liaison between communities, agencies, and other resources, services and program staff; and to do related work as required. We coordinate services and cultivate partnerships with community organizations and providers to help ensure access, organize, and deliver health care and services to people with Medi-Cal and without insurance, support resilient communities, and improve health for all.
