Nursing services are delivered in collaboration with the psychiatric care provider, ACTT team, primary care providers, pharmacies, hospitals, community partners, and each individual's identified supports, when authorized. What You'll Do: The ACTT Nurse Case Manager is an integral member of the Assertive Community Treatment Team and provides nursing, medication, physical health, care coordination, rehabilitation, and support services in community-based settings.
