Additionally, as needed, under the direction of a Registered Nurse or licensed medical provider, perform clinical functions within the scope of practice, including, but not limited to: immunizations, performance of vital signs, hearing and vision screening, and medication dispensing. Our health centers use a team-based model of care that includes case management, health educators, registered dietitians, licensed nurses, promotoras and clinical social workers to mitigate social determinants of health.
