QUALIFICATIONS: Minimum of a Bachelor’s degree in social work, psychology, counseling, rehabilitation, gerontology, sociology, or related field, plus two years of experience working with frail older adults. Field-based services are provided within the client's home setting with the goal of maintaining independent living and activating resources to allow the client to remain safely in their home setting.
